CARING FOR OUR SENIORS IN NORTH COUNTY
A unique partnership amongst four San Diego Community Health Centers
It is no secret that our population is rapidly aging and that our families are comprised of multiple generations, sometimes living in one home. By the year 2030, 1 in 4 San Diegans will be age 60 or older and the demand for comprehensive health services and support will continue to grow drastically. In an effort to meet the growing complex healthcare needs of our aging constituents in North County, a unique partnership has developed between Vista Community Clinic, TrueCare, Neighborhood Healthcare and San Ysidro Health. We are excited to introduce the Program of All Inclusive Care for the Elderly (SD PACE) as a new resource for our North County seniors and their caregivers at 1840 West Drive, Vista, California 92083.
San Diego PACE delivers high quality, compassionate care to adults aged 55 and older. Services are provided and coordinated by a team of geriatric specialized health care professionals who are bilingual and highly experienced in senior care. SD PACE is also a comprehensive health plan and provides a full range of senior services so our loved ones can age independently in the comfort of their homes. At San Diego PACE, their mission is to ensure seniors get the care they need to lead healthy, happy and independent lives.
The SD PACE interdisciplinary team works together with seniors and their families to create a coordinated personal care plan – scheduling appointments, filling prescriptions, and providing transportation to and from the SD PACE center in Vista. While at the center, seniors receive a nutritious hot meal, lots of positive social interaction, and recreational activities, in addition to medical clinic check-ups, physical therapy, and/or rehabilitative therapy.
We believe all elders deserve access to the resources they need to thrive. SD PACE focuses on the physical, mental, and social health of their patients. Helping them to age at home with a higher quality of life, giving much needed respite to their caregivers and providing outreach to isolated seniors who might otherwise have no one to turn to is their priority.
Through the impact of this special partnership, San Ysidro Health is offering senior San Diegans and their families in North County a resource that will help them thrive in a new, state of the art facility encouraging socialization, support and access to comprehensive and personalized medical care.
